Volunteer Liability Waiver and Release of Liability Agreement

This Liability Waiver and Release of Liability Agreement ("Agreement") is entered into between Unity and Pride Network ("Organization") and the undersigned volunteer ("Volunteer").

1. Acknowledgment and Assumption of Risk

I, the Volunteer, acknowledge that I have voluntarily applied to participate in volunteer activities with the Organization. I am aware that the activities may include, but are not limited to, community outreach, educational programs, mental health support, legal assistance, healthcare services, and other tasks that involve interaction with other volunteers and the public. I understand that these activities may involve risks, including but not limited to, emotional stress, exposure to sensitive information, and interactions with diverse populations.

I am voluntarily participating in these activities with full knowledge of the inherent risks and accept all risks of injury, illness, or emotional distress, including those that may result from the negligence of the Organization or its staff, volunteers, or agents.

2. Release and Waiver of Liability

In consideration for being allowed to participate in the volunteer activities, I hereby release, waive, and discharge the Organization, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, or causes of action that I may hereafter have for injuries, illnesses, damages, or losses arising out of or connected with my participation in the volunteer activities, including, but not limited to, those arising from the negligent acts or omissions of the Organization or its staff, volunteers, or agents.

3. Indemnification

I agree to indemnify and hold harmless the Organization, its directors, officers, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action, including attorneys' fees and costs, arising out of or related to my participation in the volunteer activities.

4. Medical Treatment

I understand that the Organization does not assume any responsibility for or obligation to provide financial assistance or other assistance, including, but not limited to, medical, health, or disability insurance. I hereby release and forever discharge the Organization from any claim whatsoever that arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my participation in the volunteer activities.

5. Confidentiality

I agree to maintain the confidentiality of any proprietary or sensitive information to which I may have access during my volunteer activities and not to disclose such information to any third party without the prior written consent of the Organization. This includes adherence to any Non-Disclosure Agreements (NDAs) that may be presented to me during my volunteer activities. I understand that I am bound to protect the privacy and confidentiality of all individuals within the diverse community I interact with. I agree not to speak about or disclose any information related to these individuals without their explicit consent. Failure to comply with these confidentiality requirements and NDAs may result in legal action and/or termination of my volunteer position.

6. Media Release

I grant the Organization permission to use my likeness in photographs, videos, or other media taken during my volunteer activities for promotional or other purposes, without compensation or other consideration.

7. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the state/province in which the Organization is located, without regard to its conflict of law principles.

8. Entire Agreement

This Agreement constitutes the entire agreement between the Organization and me regarding the subject matter hereof and supersedes any and all prior or contemporaneous oral or written agreements or understandings.

I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
